Important announcement regarding how to fill the electronic form for issuing the new passport.

According to the new directives of the Directorate General of Passports that Iraq Ambassad received recently about how to get a new passport (version A), so it was decided from 15/2/2010 to apply for the passport through a passport application form (Electronic form) filled by the applicant himself/herself and through the Internet.

 "You can Fill the electronic form either in (Arabic - English) language or in (Kurdish - English) language"

Directives and instructions to be followed when you fill the application form for machine-readable passport.

1. Click on this link: http://www.iraqinationality.gov.iq/eForm/Default.aspx

2. After the appearance of the electronic form, go to paragraph (place of demand) on the left side of the form and choose (outside Iraq).

3. don`t fill anything in the field (national number)

4. fill the information field (name) accurately, and then press (insert the names in English) to translate the information in the name field into the English language, in the case that the name`s characters in English is not identical with whats written in the Swedish identity, then applicant can change some character of the name as long as it will be the same arabic pronouncing. But applicant doesn’t allowed to change any arabic letter in his/her name.

5. In the absence of the surname in the iraqi ID card then type the third name (grandfather's name) in the name blank.

6. Fill in the field (personal information), date of birth in this way (day - month - year).

7. For the address field, fill in only the paragraph (the adress outside of Iraq), write the country name and the city name and street number with the building where you live.

8. Fill in the field of ID (identity card) and the certificate of nationality.

9. Don`t fill the field of housing and rationing card.

10. Fill in the field (previous passport data), Write the number of previous passport, if you have an old passport, and write the date of issue, and leave the field empty if there is no previous passport.

11. If the applicant is a less than 18 years old, then he/she should  write the full name of the guardian in the specific field.

12. don`t fill the field (Financial statements)

13. The form should be filled accurately and literally according to the information in the ID (identity card) otherwise the application will be rejected.

14. Print the form after beeing sure that you filled the right information in it.

15. the applicant have to attend to the consulate personally at the pre-installed date with the printed form and having with him/her all the supporting documents (identity card, with a clear colored copy  + Certificate of Iraqi nationality, with a clear colored copy - your previous passport, if any, with a clear colored copy) with bringing 4 photos modern color with a white background measuring 45 x 35.

16. Bring a color copy of the old Iraqi passport, if any, and from any eddition was

17. Please bring (PERSONBEVIS) in order to  prove the date of entry into Sweden, or the country in which you have residency in it.

18. Fees for the issuance of the passport is 200 SEK and have to be paid at the consulate department to the official accounts.

19. Adopt a certificate of Iraqi nationality`s father for his sons or daughters who were under the age of (12) years.

20. The identity card will not be adopted if it issued ten years ago and it will not be adopted also if it does not have a picture.

21. If you have a passport from the edition (G) befor this application, then you cannot apply for a new passport unless the last passport is expired or its pages became full with stamps.

22. In case the passport became lost or theft,  you should in this case report it at the police station, in the country where live and legilize that report from the swedish foreign ministery, and publicate the lost or theft in a newspaper to obtain the regulatory approvals from the iraqi passports authority for the purpose of issuing a new passport.

23. in case the passport damaged then the Embassy can not issue a new passport but the Embassy can only issue a laissez-passer in that case so the owner for the damaged passport can travel  to the Directorate of Passports in Baghdad so he/she can issue a new passport .

25. Recall that the period of validity of the passport is eight (8) years.
